A new waiver and consent form must be signed if you came to the park before the 2010 - 2011 season. If you came last year and signed a new waiver, you're all set to go.
There are 3 forms. Everyone must fill out 2 forms -- one waiver and one consent form. Pick the waiver for your age group.
- Click on, open, print out, fill in, and sign the waiver that matches your age group.
- Click on, open, print out, fill in, and sign a consent form.
- Bring the filled out forms to the park.
Riders Under 18 not accompanied by a parent or legal guardian must have their waiver notarized. No exceptions.
Riders under the age of 16 MUST be accompanied by an adult. That adult must remain present inside the park the entire duration of the visit.
Plan ahead. Print out, fill in, and sign a waiver and consent form in advance and save your time at the park for riding.
Remember -- everyone must fill out 2 forms -- one waiver and one consent form.
Note: RAYSMTB CLE (Cleveland) and RAYSMTB MKE (Milwaukee) have different Waivers.
Go to the respective park's website for the right waiver.
If you visit both parks, you will need to fill out park specific waivers.
